Registration for Use of Social Hall

Please note: With the approval of our Medical Advisory Committee, we now allow 10 members to sign up for one time slot together if desired. However, each household must sign the waiver. 

Please register to sign up for a small group in the social hall.  If you have questions or need help completing the form, please contact the office at 847-674-3473 or by email at office@svaj.org.

Some potential uses: 
  • Social distance Chevruta learning
  • Walking Laps 
  • Book discussion 
  • Playdate for 2-3 families 
  • Ping-pong tournament 
  • Crafting 
  • Small group stretching/movement
  • Knitting
  • Hanging out
To complete registration, you must confirm that you meet the following criteria:
  • I am a member of Skokie Valley Agudath Jacob.
  • If I have traveled out of the state of IL in the last 14 days, I will notify Rav Ari and/or the medical committee.
  • I agree to wear a mask at all times while in the building.
  • I agree to follow all Skokie Valley guidelines.
  • I agree to self assess before attending any Skokie Valley event (see below).
  • I understand that attending any prayer service, class or other program at Skokie Valley is my choice and I release Skokie Valley Agudath Jacob from any and all liability for unintentional exposure to viral or bacterial infection.
Self Assessment:

Do you or anyone in your household have common symptoms of COVID-19 (select all that apply):

a.      Yes, Cough (new or worsening)

b.      Yes, Sore throat

c.      Yes, Shortness of breath

d.      Yes, Temperature > 100 F

e.      Yes,  Loss of smell or taste

f.       NONE of the above

Have you taken any fever reducing medication within the last 24 hours?

a.      A. Yes

b.      B. No

If you answered YES to any of the above you must stay home from shul and see your physician for COVID and other testing

Anyone who is at all sick or has a fever may not attend in-person activities at Skokie Valley until cleared by a doctor and 72 hours have passed with no symptoms.

Anyone who has tested positive for COVID-19, or who has had direct contact with someone who tested positive, may not participate in any in-person activities at Skokie Valley for at least 14 days.  Please note: the last 72 hours of the 14 day period must be symptom-free, which may require an extension of the 14 day period. Anyone who has previously tested positive must also be cleared by a doctor for participation in in-person activities.


If you are over 65 or have an underlying medical condition, we strongly urge you to consult your physician before attending.
